On October 28, 2002, Jon D. Uman underwent a routine, out-patient endoscopy performed by Dr. Wolfsen. Unfortunately, and for reasons still unknown, Dr Wolfsen and his nursing staff administered a huge overdose of Versed, Demerol, and concentrated iodine causing immediate, acute, and permanent kidney failure and damage, liver failure and damage, pancreatic failure and damage, and other multi-system failures and damages. After an extensive hospital stay, and despite the odds against him, Jon D. Uman survived. When he later began to investigate what went wrong, he learned several interesting things: Although Dr. Wolfsen and the nursing staff at Mayo Clinic Jacksonville recorded the overdose of Versed and Demerol, there was no record of who, when, or how the overdose occurred. There was no record of the overdose of iodine concentrate. Most disturbing, there was no record of vital signs for more than 30 minutes during the procedure. Accordingly, it is still unknown whether Jon D. Uman suffered shock, respiratory distress, or heart failure during the procedure.
